Ante el riesgo que suponen, los microbi\u00f3logos publican descripciones detalladas para alertar al resto de la comunidad m\u00e9dica y cient\u00edfica. <\/span><!--more--><\/p>\n<p><span style=\"font-family: Calibri, serif\">ST147 hace referencia al clon. Las letras <\/span><span style=\"font-family: Calibri, serif\"><b>ST<\/b><\/span><span style=\"font-family: Calibri, serif\"> indican que el clon se identifica mediante secuenciaci\u00f3n de ADN con el m\u00e9todo <\/span><span style=\"font-family: Calibri, serif\"><b>MLST<\/b><\/span><span style=\"font-family: Calibri, serif\"> (<\/span><a href=\"https:\/\/pubmlst.org\/multilocus-sequence-typing\"><span style=\"color: #000080\"><u><span style=\"font-family: Calibri, serif\"><i><b>M<\/b><\/i><\/span><\/u><\/span><span style=\"color: #000080\"><u><span style=\"font-family: Calibri, serif\"><i>ulti<\/i><\/span><\/u><\/span><span style=\"color: #000080\"><u><span style=\"font-family: Calibri, serif\"><i><b>l<\/b><\/i><\/span><\/u><\/span><span style=\"color: #000080\"><u><span style=\"font-family: Calibri, serif\"><i>ocus <\/i><\/span><\/u><\/span><span style=\"color: #000080\"><u><span style=\"font-family: Calibri, serif\"><i><b>s<\/b><\/i><\/span><\/u><\/span><span style=\"color: #000080\"><u><span style=\"font-family: Calibri, serif\"><i>equence <\/i><\/span><\/u><\/span><span style=\"color: #000080\"><u><span style=\"font-family: Calibri, serif\"><i><b>t<\/b><\/i><\/span><\/u><\/span><span style=\"color: #000080\"><u><span style=\"font-family: Calibri, serif\"><i>yping<\/i><\/span><\/u><\/span><\/a><span style=\"font-family: Calibri, serif\">), que clasifica los linajes en <\/span><span style=\"font-family: Calibri, serif\"><b>s<\/b><\/span><span style=\"font-family: Calibri, serif\">ecuencio<\/span><span style=\"font-family: Calibri, serif\"><b>t<\/b><\/span><span style=\"font-family: Calibri, serif\">ipos; el n\u00famero lo identifica espec\u00edficamente como el secuenciotipo 147 de una <\/span><span style=\"color: #000080\"><u><a href=\"https:\/\/bigsdb.pasteur.fr\/klebsiella\/\"><span style=\"font-family: Calibri, serif\">base de datos <\/span><\/a><\/u><\/span><span style=\"font-family: Calibri, serif\">que recoge secuenciotipos de todo el mundo obtenidos por el mismo m\u00e9todo<\/span><span style=\"font-family: Calibri, serif\">. Por su parte, NDM-14 es una enzima <\/span><span style=\"font-family: Calibri, serif\">\u2014<\/span><span style=\"font-family: Calibri, serif\">en concreto, una betalactamasa<\/span><span style=\"font-family: Calibri, serif\">\u2014<\/span><span style=\"font-family: Calibri, serif\"> que confiere resistencia a los antibi\u00f3ticos betalact\u00e1micos (los de la familia de la penicilina).<\/span><\/p>\n<p><span style=\"font-family: Calibri, serif\">ST147 es lo que se conoce como un <\/span><span style=\"font-family: Calibri, serif\"><b>clon de alto riesgo<\/b><\/span><span style=\"font-family: Calibri, serif\"> de <\/span><span style=\"font-family: Calibri, serif\"><i>K. pneumoniae<\/i><\/span><span style=\"font-family: Calibri, serif\">. Estos clones son estirpes multirresistentes de distribuci\u00f3n mundial que suelen asociarse a brotes hospitalarios. Existen otros clones de alto riesgo <\/span><span style=\"font-family: Calibri, serif\">\u2014como <\/span><span style=\"font-family: Calibri, serif\">ST11, <\/span><a href=\"https:\/\/www.madrimasd.org\/blogs\/microbiologia\/2019\/12\/15\/132484\"><span style=\"color: #000080\"><u><span style=\"font-family: Calibri, serif\">ST258 (el <\/span><\/u><\/span><span style=\"color: #000080\"><u><span style=\"font-family: Calibri, serif\"><i>Indominus<\/i><\/span><\/u><\/span><span style=\"color: #000080\"><u><span style=\"font-family: Calibri, serif\">)<\/span><\/u><\/span><\/a><span style=\"font-family: Calibri, serif\">, ST307 o ST405<\/span><span style=\"font-family: Calibri, serif\">\u2014,<\/span><span style=\"font-family: Calibri, serif\"> cada uno con su historia particular. El sistema actual de identificaci\u00f3n de clones de <\/span><span style=\"font-family: Calibri, serif\"><i>K. pneumoniae<\/i><\/span> <span style=\"color: #000080\"><u><a href=\"https:\/\/journals.asm.org\/doi\/10.1128\/jcm.43.8.4178-4182.2005\"><span style=\"font-family: Calibri, serif\">se public\u00f3 en 2005<\/span><\/a><\/u><\/span><span style=\"font-family: Calibri, serif\">, y por tanto es a partir de esa fecha cuando podemos seguirlos. Sin embargo, los estudios de cepas almacenadas y los an\u00e1lisis filogen\u00e9ticos sit\u00faan el origen de la mayor\u00eda de estos clones en las d\u00e9cadas de los ochenta y noventa del siglo pasado. El origen de ST147, en particular, se remonta a la d\u00e9cada de los cincuenta, coincidiendo con el desarrollo de la medicina hospitalaria moderna y el uso extensivo de antibi\u00f3ticos. Los clones m\u00e1s estrechamente relacionados est\u00e1n unidos por l\u00edneas y el di\u00e1metro de los c\u00edrculos es proporcional a la frecuencia del clon. PLoS One. 2013;8(2):e57091. doi: <a href=\"https:\/\/dx.plos.org\/10.1371\/journal.pone.0057091\">10.1371\/journal.pone.0057091<\/a>.<\/figcaption><\/figure>\n<p>&nbsp;<\/p>\n<p><span style=\"font-family: Calibri, serif\">NDM-14, por su parte, es una metalo-beta-lactamasa; es decir, una enzima con actividad beta-lactamasa dependiente de metales (en concreto de iones de zinc). Estas enzimas tienen la capacidad de romper el anillo beta-lact\u00e1mico de los antibi\u00f3ticos de la familia de las penicilinas, inactiv\u00e1ndolos. Por lo tanto, las bacterias que producen beta-lactamasas son resistentes a los antibi\u00f3ticos del grupo de la penicilina y sus derivados. Las NDM, en particular, pertenecen a las carbapenemasas, que son capaces de hidrolizar tanto los carbapen\u00e9micos (antibi\u00f3ticos hospitalarios de \u00faltimo recurso) como a casi todos los beta-lact\u00e1micos de generaciones anteriores (penicilinas y cefalosporinas). La variante original, <\/span><span style=\"color: #000080\"><u><a href=\"https:\/\/www.madrimasd.org\/blogs\/microbiologia\/2010\/09\/08\/130979\"><span style=\"font-family: Calibri, serif\">NDM-1, se describi\u00f3 en 2010<\/span><\/a><\/u><\/span><span style=\"font-family: Calibri, serif\">, y desde entonces se ha expandido por todo el mundo, evolucionando y generando hasta hoy decenas de nuevas variantes. NDM-14 es una de ellas.<\/span><\/p>\n<p><span style=\"font-family: Calibri, serif\">El clon ST147 se describi\u00f3 en 2008 en Hungr\u00eda portando una <\/span><span style=\"font-family: Calibri, serif\"><i>betalactamasa de espectro extendido<\/i><\/span><span style=\"font-family: Calibri, serif\"> (BLEE, otra familia de estas enzimas). <\/span><span style=\"font-family: Calibri, serif\">Poco despu\u00e9s, e<\/span><span style=\"font-family: Calibri, serif\">n 2011 se detect<\/span><span style=\"font-family: Calibri, serif\">aron cepas portadoras de<\/span><span style=\"font-family: Calibri, serif\"> carbapenemasas <\/span><span style=\"font-family: Calibri, serif\">en varios pa\u00edses<\/span><span style=\"font-family: Calibri, serif\">. Actualmente, se notifican con frecuencia brotes de ST147 <\/span><span style=\"font-family: Calibri, serif\">con<\/span><span style=\"font-family: Calibri, serif\"> diferentes carbapenemasas en todo el mundo; en general asociado<\/span><span style=\"font-family: Calibri, serif\">s<\/span><span style=\"font-family: Calibri, serif\"> a brotes hospitalarios, pero cada vez es m\u00e1s com\u00fan <\/span><span style=\"color: #000080\"><u><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/39612983\/\"><span style=\"font-family: Calibri, serif\">en medios extrahospitalarios<\/span><\/a><\/u><\/span><span style=\"font-family: Calibri, serif\">. En Espa\u00f1a se han descrito casos <\/span><span style=\"font-family: Calibri, serif\">y brotes<\/span><span style=\"font-family: Calibri, serif\"> de ST147 con otras carbapenemasas como, <\/span><span style=\"font-family: Calibri, serif\">por ejemplo,<\/span> <span style=\"color: #000080\"><u><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/40407340\/\"><span style=\"font-family: Calibri, serif\">KPC-3 en Andaluc\u00eda<\/span><\/a><\/u><\/span><span style=\"font-family: Calibri, serif\">, <\/span><span style=\"color: #000080\"><u><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/38276157\/\"><span style=\"font-family: Calibri, serif\">OXA-48 en Asturias<\/span><\/a><\/u><\/span><span style=\"font-family: Calibri, serif\">, <\/span><span style=\"color: #000080\"><u><a href=\"https:\/\/academic.oup.com\/jac\/article\/74\/12\/3489\/5559588?login=true\"><span style=\"font-family: Calibri, serif\">NDM-1 en Galicia y Catalu\u00f1a<\/span><\/a><\/u><\/span><span style=\"font-family: Calibri, serif\"> y <\/span><span style=\"color: #000080\"><u><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/38996870\/\"><span style=\"font-family: Calibri, serif\">OXA-48\/NDM-1 en Madrid<\/span><\/a><\/u><\/span><span style=\"font-family: Calibri, serif\">.<\/span><\/p>\n<p><span style=\"font-family: Calibri, serif\">Y as\u00ed llegamos al panorama actual. La bacteria llega a una poblaci\u00f3n susceptible <\/span><span style=\"font-family: Calibri, serif\">\u2014los pacientes de <\/span><span style=\"font-family: Calibri, serif\">un hospital<\/span><span style=\"font-family: Calibri, serif\">\u2014<\/span><span style=\"font-family: Calibri, serif\"> en la que se expande clonalmente <\/span><span style=\"font-family: Calibri, serif\">y se amplifica<\/span><span style=\"font-family: Calibri, serif\">. En ese entorno entra en contacto con otros clones y especies con los que intercambia genes de resistencia y salta a otra poblaci\u00f3n en la que el ciclo se repite.
