No es que las cifras estuvieran subestimadas, es que no exist\u00edan ni datos reales ni estimados para hacernos una idea de lo que estaba pasando al menos en Europa.<o:p><\/o:p><\/span><\/p>\n<p class=\"MsoNormal\" style=\"text-align: justify;\"><span style=\"font-size: 11pt; font-family: &quot;Comic Sans MS&quot;;\" lang=\"ES\"><o:p>&nbsp;<\/o:p><\/span><\/p>\n<p class=\"MsoNormal\" style=\"text-align: justify;\"><span style=\"font-size: 11pt; font-family: &quot;Comic Sans MS&quot;;\" lang=\"ES\">Hace 2 semanas empiezan a dar nuevos datos y nos explican que en la primera semana, mediante la Red de M\u00e9dicos Centinelas, se han registrado m\u00e1s 12.000 casos nuevos de Gripe A H1 N1 y en la semana pasada los casos han sido unos 14.000 y esto ya se aproxima m\u00e1s a lo que est\u00e1 sucediendo en realidad.<o:p><\/o:p><\/span><\/p>\n<p class=\"MsoNormal\" style=\"text-align: justify;\"><span style=\"font-size: 11pt; font-family: &quot;Comic Sans MS&quot;;\" lang=\"ES\"><o:p>&nbsp;<\/o:p><\/span><\/p>\n<p class=\"MsoNormal\" style=\"text-align: justify;\"><span style=\"font-size: 11pt; font-family: &quot;Comic Sans MS&quot;;\" lang=\"ES\"><a href=\"http:\/\/www.elpais.com\/articulo\/sociedad\/Espana\/detecta\/12270\/casos\/nueva\/gripe\/semana\/elpepusocsal\/20090807elpepusoc_12\/Tes\">Los medios de comunicaci\u00f3n nos dan las cifras y nos cuentan un batiburrillo de supuestos modelos matem\u00e1ticos con los que se estar\u00edan calculando estas cifras<\/a>. En Madrid, que es el caso que conozco a fondo, est\u00e1bamos padeciendo brotes sucesivos de sarampi\u00f3n y el sistema de vigilancia tradicional (EDO o Enfermedades de Declaraci\u00f3n Obligatoria) no nos suministraba datos pormenorizados para poder controlarlos a tiempo, necesit\u00e1bamos un sistema m\u00e1s \u00e1gil, con un conjunto b\u00e1sico de datos de los pacientes (edad, sexo, etc) que, si fuera necesario, pudieran recoger muestras biol\u00f3gicas para caracterizar mejor los brotes.<o:p><\/o:p><\/SPAN><\/P> <P class=MsoNormal style=\"TEXT-ALIGN: justify\"><SPAN lang=ES style=\"FONT-SIZE: 11pt; FONT-FAMILY: 'Comic Sans MS'\"><o:p>&nbsp;<\/o:p><\/SPAN><\/P> <P class=MsoNormal style=\"TEXT-ALIGN: justify\"><SPAN lang=ES style=\"FONT-SIZE: 11pt; FONT-FAMILY: 'Comic Sans MS'\"><SPAN>&nbsp;<\/SPAN><o:p><\/o:p><\/SPAN><\/P> <P class=MsoNormal style=\"TEXT-ALIGN: justify\"><SPAN lang=ES style=\"FONT-SIZE: 11pt; FONT-FAMILY: 'Comic Sans MS'\">La &nbsp;Red centinela sanitaria consiste en un <B><SPAN style=\"COLOR: blue\">sistema espec\u00edfico de informaci\u00f3n, que se basa en la colaboraci\u00f3n voluntaria y activa de profesionales sanitarios (m\u00e9dicos y pediatras) del Sistema de Atenci\u00f3n Primaria de Salud.&nbsp; <o:p><\/o:p><\/SPAN><\/B><\/SPAN><\/P> <P class=MsoNormal style=\"TEXT-ALIGN: justify\"><SPAN lang=ES style=\"FONT-SIZE: 11pt; FONT-FAMILY: 'Comic Sans MS'\">La dificultad, si es que tiene alguna, es el dise\u00f1o de la red <B><SPAN style=\"COLOR: blue\">porque las poblaciones de los m\u00e9dicos que participan han de ser representativas de la poblaci\u00f3n general<\/SPAN><\/B> para poder llevar a cabo inferencias poblacionales representativas de toda la comunidad. En Madrid el dise\u00f1o se realiz\u00f3 teniendo en cuenta <B><SPAN style=\"COLOR: blue\">variables demogr\u00e1ficas, socioculturales, sociolaborales y sanitarias para a continuaci\u00f3n, mediante un<\/SPAN><\/B><\/SPAN><B><SPAN lang=ES style=\"FONT-SIZE: 10pt; COLOR: blue; FONT-FAMILY: Arial\"> <\/SPAN><\/B><B><SPAN lang=ES style=\"FONT-SIZE: 11pt; COLOR: blue; FONT-FAMILY: 'Comic Sans MS'\">an\u00e1lisis de cluster,<\/SPAN><\/B><SPAN lang=ES style=\"FONT-SIZE: 11pt; FONT-FAMILY: 'Comic Sans MS'\"> obtener ocho conglomerados de distritos sanitarios, en los que se solicit\u00f3 la <B><SPAN style=\"COLOR: blue\">colaboraci\u00f3n voluntaria de los m\u00e9dicos<\/SPAN><\/B>.<\/SPAN><SPAN lang=ES style=\"FONT-SIZE: 11pt; FONT-FAMILY: 'Comic Sans MS'\"><o:p><\/o:p><\/SPAN><\/P> <P class=MsoNormal style=\"TEXT-ALIGN: justify\"><SPAN lang=ES style=\"FONT-SIZE: 11pt; FONT-FAMILY: 'Comic Sans MS'\">En la actualidad colaboran como notificadores 40 m\u00e9dicos generalistas y 33 pediatras de atenci\u00f3n primaria. <\/SPAN><SPAN lang=ES style=\"FONT-SIZE: 11pt; FONT-FAMILY: 'Comic Sans MS'\"><o:p><\/o:p><\/SPAN><\/P> <P class=MsoNormal style=\"TEXT-ALIGN: justify\"><SPAN lang=ES style=\"FONT-SIZE: 11pt; FONT-FAMILY: 'Comic Sans MS'\">En resumen:&nbsp; <o:p><\/o:p><\/SPAN><\/P> <UL style=\"MARGIN-TOP: 0in\" type=square> <LI class=MsoNormal style=\"COLOR: blue; TEXT-ALIGN: justify\"><SPAN lang=ES style=\"FONT-SIZE: 11pt; FONT-FAMILY: 'Comic Sans MS'\">Participaci\u00f3n voluntaria de los m\u00e9dicos. <o:p><\/o:p><\/SPAN> <LI class=MsoNormal style=\"COLOR: blue; TEXT-ALIGN: justify\"><SPAN lang=ES style=\"FONT-SIZE: 11pt; FONT-FAMILY: 'Comic Sans MS'\">Selecci\u00f3n de los m\u00e9dicos y pediatras entre los voluntarios. <o:p><\/o:p><\/SPAN> <LI class=MsoNormal style=\"COLOR: blue; TEXT-ALIGN: justify\"><SPAN lang=ES style=\"FONT-SIZE: 11pt; FONT-FAMILY: 'Comic Sans MS'\">Que estos m\u00e9dicos seleccionados sean representativos de la poblaci\u00f3n atendiendo a criterios demogr\u00e1ficos, socioculturales y sociolaborales. <o:p><\/o:p><\/SPAN> <LI class=MsoNormal style=\"COLOR: blue; TEXT-ALIGN: justify\"><SPAN lang=ES style=\"FONT-SIZE: 11pt; FONT-FAMILY: 'Comic Sans MS'\">Transmisi\u00f3n semanal de la informaci\u00f3n. Sistema de retroinformaci\u00f3n y difusi\u00f3n de resultados.<o:p><\/o:p><\/SPAN> <LI class=MsoNormal style=\"COLOR: blue; TEXT-ALIGN: justify\"><SPAN lang=ES style=\"FONT-SIZE: 11pt; FONT-FAMILY: 'Comic Sans MS'\">Participaci\u00f3n de los profesionales sanitarios en la coordinaci\u00f3n y gesti\u00f3n de la Red.<o:p><\/o:p><\/SPAN> <LI class=MsoNormal style=\"COLOR: blue; TEXT-ALIGN: justify\"><SPAN lang=ES style=\"FONT-SIZE: 11pt; FONT-FAMILY: 'Comic Sans MS'\">Reconocimiento y motivaci\u00f3n de los profesionales que participan en la misma. <o:p><\/o:p><\/SPAN><\/LI><\/UL> <P class=MsoNormal style=\"TEXT-ALIGN: justify\"><SPAN lang=ES style=\"FONT-SIZE: 11pt; FONT-FAMILY: 'Comic Sans MS'\">&nbsp;&nbsp;&nbsp; <o:p><\/o:p><\/SPAN><\/P> <P class=MsoNormal style=\"TEXT-ALIGN: justify\"><SPAN lang=ES style=\"FONT-SIZE: 11pt; FONT-FAMILY: 'Comic Sans MS'\"><BR>Las enfermedades estudiadas han de tener unas caracter\u00edsticas muy espec\u00edficas como:<o:p><\/o:p><\/SPAN><\/P> <UL style=\"MARGIN-TOP: 0in\" type=square> <LI class=MsoNormal style=\"COLOR: blue; TEXT-ALIGN: justify\"><SPAN lang=ES style=\"FONT-SIZE: 11pt; FONT-FAMILY: 'Comic Sans MS'\">Ser de f\u00e1cil identificaci\u00f3n en AP por anamnesis, exploraci\u00f3n f\u00edsica o pruebas complementarias accesibles. <o:p><\/o:p><\/SPAN> <LI class=MsoNormal style=\"COLOR: blue; TEXT-ALIGN: justify\"><SPAN lang=ES style=\"FONT-SIZE: 11pt; FONT-FAMILY: 'Comic Sans MS'\">Se ha de disponer de una definici\u00f3n clara y estandarizada de cada enfermedad. <o:p><\/o:p><\/SPAN> <LI class=MsoNormal style=\"COLOR: blue; TEXT-ALIGN: justify\"><SPAN lang=ES style=\"FONT-SIZE: 11pt; FONT-FAMILY: 'Comic Sans MS'\">El problema de salud a vigilar debe ser considerado prioritario en la CCAA correspondiente.<o:p><\/o:p><\/SPAN> <LI class=MsoNormal style=\"COLOR: blue; TEXT-ALIGN: justify\"><SPAN lang=ES style=\"FONT-SIZE: 11pt; FONT-FAMILY: 'Comic Sans MS'\">La enfermedad debe tener una frecuencia de aparici\u00f3n m\u00ednima en Atenci\u00f3n Primaria.<o:p><\/o:p><\/SPAN><\/LI><\/UL> <P class=MsoNormal style=\"TEXT-ALIGN: justify\"><SPAN lang=ES style=\"FONT-SIZE: 11pt; COLOR: blue; FONT-FAMILY: 'Comic Sans MS'\"><o:p>&nbsp;<\/o:p><\/SPAN><\/P> <P class=MsoNormal style=\"TEXT-ALIGN: justify\"><SPAN lang=ES style=\"FONT-SIZE: 11pt; FONT-FAMILY: 'Comic Sans MS'\">Ahora vamos a poner un ejemplo que todos conocemos para entender mejor este sistema: el dise\u00f1o ser\u00eda equivalente a los estudios de audiencia que dan los medios de comunicaci\u00f3n.
