La Higiene oral en la tercera edad disminuye la incidencia y mortalidad por neumonía
Siempre me ha parecido muy importante mantener una adecuada higiene buco dental, parece que a estas alturas del siglo XXI ya es un hábito que ha penetrado, felizmente, en los hábitos de la sociedad y todos cuidamos nuestros dientes acudiendo al odontólogo cuando sentimos alguna molestia, o simplemente para nuestras revisiones periódicas aunque no exista ninguna molestia y preservamos este buen estado de salud buco dental diariamente con los cepillados tras cada comida, el hilo dental y los colutorios. Por eso me parecieron muy interesantes las indicaciones del Center for Diseases Control que plasmé en el post “higiene dental desde el primer diente”
Cuando descubrí este artículo me pareció muy importante recalcar que mediante la adquisición de estos hábitos y manteniéndolos durante toda la vida se pueden prevenir muchas más enfermedades.
En este caso se trata de la prevención de las neumonías, tan frecuentes en los ancianos que pueden provocar un exceso de mortalidad prematura, simplemente ayudando a las personas más incapacitadas a mantener una buena higiene oral.
La neumonía es la principal causa de muerte en los residentes ancianos
de las Residencias de personas mayores, señalaron
colaboradores, en el estudio. «Diversas investigaciones han
demostrado que una deficiente higiene oral o un cuidado inadecuado de la
cavidad oral también están asociados con la ocurrencia de neumonía,».

Society cuyo resumen podéis consultar en Medline Plus a través del siguiente enlace.
Como todavía no he podido acceder al estudio completo hago una interpretación de los principales resultados.
Al parecer realizaron un estudio de casos y controles siendo los casos los internados en una Residencia de personas mayores, de mayor edad y con gran discapacidad y los controles las personas internadas en otras alas de la misma residencia más jóvenes y con menos discapacidad.
A los más mayores se les asignó personal de apoyo que les proporcionaba ayuda en su higiene oral y en los controles no se prestó ninguna ayuda adicional.
Y cito textualmente los resultados:
Inicialmente, no se registró ninguna diferencia en la tasa de
mortalidad por neumonía entre los dos grupos.
Sin embargo, los pacientes en el grupo que recibió el cuidado oral apropiado fueron de mayor edad y estuvieron con mayor discapacidad en comparación con los que no recibieron dicha atención; y una vez que se consideró este
factor, el riesgo de morir a causa de neumonía fue tres veces mayor en
los pacientes que no recibieron el cuidado oral.
La neumonía en los ancianos a menudo es desencadenada por la
aspiración de saliva o de alimentos. Es probable que el riesgo para
desarrollar neumonía «dependa de la calidad y de la cantidad del
contenido orofaríngeo de un paciente en el momento de la
introducción en el sistema respiratorio,» explicaron
sus colaboradores.
«La cantidad de saliva inhalada y una predisposición para sufrir
eventos de aspiración masiva no se modifican con el cuidado oral;»
«pero el estudio indica que el cuidado de la cavidad oral
puede estar involucrado en reducir significativamente las
características dañinas del ambiente intra-oral, disminuyendo el
riesgo para que un paciente fallezca a causa de neumonía.»
Creo que esta sencilla medida puede no solo disminuir la incidencia y mortalidad por neumonía en las personas mayores sino también mejorar la calidad de vida de las mismas.
