Endemia, brote, epidemia, pandemia. Aclarando algunos términos
Veamos las definiciones del libro “el control de las enfermedades transmisibles” (OPS 18 edición. Publicación Científica y Técnica Nº 613).
Endemia:
Término que denota la presencia habitual de una enfermedad o un agente infeccioso en una determinada zona geográfica o grupo de población.
Prevalencia habitual de una enfermedad determinada en dicha zona.
Es como la medida basal de la enfermedad en una zona geográfica más o menos extensa.
Otras definiciones:
Se entiende por Endemia la existencia en un área o región de una enfermedad en una proporción no significativa sobre el número de casos habituales en ella. Cuando la presencia de una enfermedad, tanto transmisible como no transmisible, se da de una forma regular se dice que esa enfermedad existe con carácter endémico.

La endemia (del griego E?d?µ?a, que afecta a un país) es un proceso patológico permanente en una zona geográfica; generalmente infeccioso, pero no siempre. En este caso, la enfermedad se mantiene en un nivel usual, incluyendo sus variables estacionales.
Principalmente cuando una enfermedad se mantiene durante largos periodos en una zona. Enfermedad que reina habitualmente, o en épocas fijas, en un país o comarca.
La endemia se da cuando una enfermedad persiste durante varios años en un lugar y el número de individuos que la padecen no es tan elevado.
Ya vale, que hay definiciones para todos los gustos.
1.- La aparición de dos o más casos de la misma enfermedad asociados en tiempo, lugar y persona.
2.- El incremento significativo de casos en relación a los valores habitualmente observados.
3.- La agregación de casos de una enfermedad en un territorio y en un tiempo comprendido entre el mínimo y el máximo período de incubación o de latencia puede ser considerado, también, indicativo de brote.
4.- La aparición de una enfermedad, problema o riesgo para la salud en una zona hasta entonces libre de ella.
5.- La presencia de cualquier proceso relevante de intoxicación aguda colectiva, imputable a causa accidental, manipulación o consumo.
6.- La aparición de cualquier incidencia de tipo catastrófico que afecte, o pueda afectar, a la salud de la Comunidad.
Además
Un solo caso de una enfermedad contagiosa, ausente desde hace largo tiempo en la población o la primera invasión de una enfermedad que anteriormente no había hecho su aparición en esa zona requieren su inmediata notificación y una completa investigación in situ; dos casos de estas características, asociados en tiempo y lugar, pueden ser suficientes para declarar la existencia de una epidemia o brote epidémico.
Epidemia:
Aparición, en una comunidad o región definida, de casos de una enfermedad (o de un brote) con una frecuencia que claramente rebasa la incidencia normal prevista. El número de casos que indica la existencia de una epidemia varía según el agente infeccioso, las dimensiones y el tipo de la población expuesta, su experiencia previa o la falta de exposición a la enfermedad, así como la época y el lugar donde se presenta. Así pués, la epidemicidad es relativa a la frecuencia habitual de la enfermedad en la misma zona, entre la población especificada y en la misma estación del año.
No puedo resistirme a poner la definición de mi querido Diccionario de Epidemiología Last. Brote, sinónimo de epidemia, a veces constituye un término preferible al de epidemia para evitar el sensacionalismo que puede asociarse al uso de esta última palabra. Alternativamente, epidemia localizada, epidemia localizada, no generalizada.
Epidemia, del griego epi encima y demos pueblo. Aparición en una comunidad o región de casos de una enfermedad, conducta específica u otros hechos, en relación con la salud, claramente excesivos con respecto a lo que cabe esperar en condiciones normales. La comunidad o región y el periodo en que se presentan los casos se hallan netamente definidos.
Como vemos es una cuestión de matices en la que tenemos que barajar el tiempo el lugar y el número de casos, generalmente si está muy delimitado en tiempo, lugar y persona decimos que es un brote, si se va extendiendo en estas tres variables hablaríamos de epidemia. Realmente la utilización de una u otra terminología, aunque existen definiciones para casi todo, la da la experiencia.
Pandemia:
Una pandemia es la afectación de una enfermedad de personas a lo largo de un área geográficamente extensa. Técnicamente hablando debería cubrir el mundo entero y afectar a todos. Afortunadamente no ha habido una pandemia en ese sentido de
Para que una enfermedad tome la denominación de Pandemia, ésta debe tener un alto grado de infectabilidad y un fácil traslado de un sector geográfico a otro. Y no tiene nada que ver con la mortalidad o la letalidad de la enfermedad en cuestión.
